If your trip starts to gain complexity or substantial cost as you plan it, or you are a first-time cruiser, you should consider trip insurance. You usually must buy it after you have purchased your entire trip. Its cost will basically be determined by your total trip cost, the age of travelers in your group, levels of coverage, and options for coverage requested for certain problems, e.g., treatment for sickness or injury (on and off the ship), medical evacuation, basic distance of the trip from home, certain pre-existing medical conditions. Many insurance policies will cover pre-existing medical conditions if purchased within a few days of booking your trip; they often do not if bought later.
Travel experts consistently recommend getting insurance from other than airlines or cruise lines. Coverage offered by "lines" often focuses only on their responsibilities, while a quality policy will cover your end-to-end trip. Some "lines" also offer just cancellation protection, but its cost can approach that for well-purchased, basic trip coverage. You may obtain better rates and/or coverage by buying coverage through or from an association you belong to, e.g., AAA, AA.
